I am pleased that you have given Logos an opportunity to serve you and your ministry. There are many things that we can do together to facilitate the calling God has given you, and give you the peace of mind that your ministry is protected from harm because of some legal or functional oversight.

Let me go over some of the things we will need and give you a few shortcuts to help fulfill your request for membership. 

1.  If you have not already done so, you will need to incorporate your church or ministry as a not-for-profit corporation with the Secretary of State in your State of residence. This can be done on-line or by mail. The fees are nominal and the forms are self-explanatory. Even if you use an attorney it should not cost more than two or three hundred dollars. If you need a sample copy of appropriate by-laws and a sample constitution, please go to our website and click on the appropriate forms (www.logosglobalnetwork.com).

2.  You will also need an employer identification number. This to can be done online at www.tax9er.com and costs about ten dollars.

These two items must accompany your church or ministry application before it can be processed. Once your application has been approved and you have been issued your certificate and 501 (c) (3) group exemption number, your ministry will be reported to the IRS for their official listing of approved ministries. In order to remain on the list you must renew your membership promptly each year upon receipt of your invoice. If the renewal and fees are not received your ministry will be removed from our report to the IRS, and thus removed from their approved list as well.  

If you are applying for ministerial credentials please indicate in the appropriate box whether you are seeking credentials as a Commissioned Minister, Licensed Minister or Ordained Minister. Give the reference forms to the recommending persons and have them send the completed forms directly to Logos. You must renew your membership annually for your credentials to remain valid.
